Medical Information

When your child joins our school we request you complete our information sheets which will notify us if your child has any specific medical needs or allergies.  We request if the information provided at the start of the year changes, please notify us by email. office@montgomery.powys.sch.uk

There may be times that your child needs to have prescribed medication administered in the school day.  If this is the case you must complete the below medical form and hand it to a member of staff along with the medication. Please do not leave medicine in your child's bag.  A hard copy medical form can also be requested from the school office.
